A cardiac-or heart-transplant is a surgical transplant that is performed on patients with end-stange heart failure or another form of severe coronary artery disease where other medical/surgical transplants have failed. It involves taking the heart from a healthy but deceased person (i.e., someone who was otherwise healthy though died in an accident) and transplanting it into a sick person.
